Telehealth Patient Specialist presso St. Luke's Health System

St. Luke’s Health System is seeking a Telehealth Patient Specialist to join our Ambulatory Care team. In this role, you will help support patients with chronic conditions. You will be an essential part of a collaborative team focused on improving access to care, reducing hospital visits, and empowering patients to manage their health more effectively.

Onsite role

Position Details:

• Shift:3 x12-hour shifts (7:00 AM to 7:00 PM)

• Schedule: Rotating weekends and holidays required

• Pay Range: $17.32 – $19. plus, per hour, depending on experience

What You’ll Do:

• Onboard new patients into the remote patient management program

• Monitor and manage patients living with chronic conditions

• Assist patients in adhering to care plans and maintaining compliance with program guidelines

• Provide basic technical support for remote monitoring equipment

• Perform scheduling functions, similar to a clinic setting

• Utilize medical terminology and clinical knowledge to support communication between patients and care teams

Qualifications:

High school diploma or equivalent required

Preferred Qualifications:
- Experience as a Certified Nursing Assistant (CNA), Unit Clerk, or in a similar role
- Familiarity with medical terminology
- Background in a healthcare setting, ideally ambulatory or telehealth
