What You'll Do:

• Provide compassionate patient care by triaging phone calls, completing patient care visits, attending death visits, and ordering supplies and equipment.
• Lead and supervise weekend care teams, ensuring smooth operations and continuity of care.
• Conduct hospice assessments, symptom management visits, and support patient admissions.
• Triage patient and family phone calls, providing timely clinical judgment and guidance.
• Support and educate families, caregivers, and patients during critical end-of-life transitions.
• Collaborate with interdisciplinary teams to maintain consistent, compassionate care.
• Mentor nursing staff, promote team cohesion, and uphold care standards during Weekend hours.

What You Bring:

• Michigan RN license (required)
• Minimum 2 years nursing experience, 4+ years preferred 
• Hospice or leadership experience is preferred  
• Strong communication and decision-making abilities
• EMR proficiency
• Independence and clinical confidence
• BSN preferred but not required
