Title: Nurse Practitioner PRN
Job Summary
The Advanced Practice Provider, in collaboration and consultation with consulting physicians and other health care professionals, provides holistic, compassionate, medically necessary diagnostic and treatment services to patients/residents and provides care coordination and care management activities.
Essential Job Functions
• Delivers cost-effective, quality care to assigned residents.
• Expedites the diagnosis, management, and treatment of all assigned residents, including writing orders for SNF services, home health, skill-in-place services, labs, diagnostic testing, specialist consults, therapy, medications, outpatient surgeries and procedures, and hospital and ER services.
• Complies with the documentation requirements and protocols of organization and contracted payors.
• Manages both medical and behavioral chronic and acute conditions effectively in collaboration with the attending physician or specialty provider.
• Provides or directs preventive care for residents.
• Leads Interdisciplinary Care Team meetings as needed or required by payors or as requested by MJH.
• Follows the course and status of resident while in hospital or SNF and updates the MJH care team.
• Follows approved practice protocols, guidelines, critical pathways, policies and procedures.
• Works collaboratively with physicians, case managers, social workers, family members, key care givers and any appropriate ancillary medical personnel as appropriate.
• Leads culturally sensitive and appropriate end-of-life conversations with residents and their responsible parties.
• Identifies overall quality improvement activities through assessing and planning health care which is individualized to the resident.
• Promotes and maintains lines of communication between department personnel, Nursing Services, Physicians, patients, and families.
• Actively participates in family/patient conferences, as well as conferences in the areas of morbidity and mortality, pharmacy, therapeutics, and data sets.
• Develops, and provides educational programs to staff, residents and caregivers as needed.
Job Requirements
Education
• Graduate from an accredited school of nursing, as an R.N.
• Advanced training and certification as an Advanced Practice Registered Nurse (APRN)
Experience
• Minimum of 5 years of experience as an ARNP or PA
• I-SNP experience strongly preferred
• Experience with value-based care strongly preferred
Licenses/Certifications
• Current APRN license by the Florida State Board of Nursing
• Current Advanced Nurse Practitioner Certification (ANCC, AANP)
• Current ACLS Certification
Abilities required
• Excellent verbal and written communication skills
• Skills in clinical decision-making, appropriate prioritization, and time management skills
• Demonstrates a caring attitude toward MJH residents, patients, family members, employees, and other members of the care team, internal and external
Functional Demands
Environment Work Conditions
• Normal working condition with adequate lighting and ventilation
• Exposure to toxic or caustic chemicals; bio-hazardous waste; flammable, explosive gas
• Ability to assist transferring patients and ambulation which may involve lifting, pushing, and pulling of patients from 50-150 pounds
Infectious Material Exposure
• Routine exposure to blood/body fluids/tissues
