Under general supervision, this position will perform differential response and case management services to clients in accordance with the funding contract.  This may include, but is not limited to needs assessments, referrals to community resources, one-on-one coaching, CFT coordination/participation, follow-up contact, up to 25 home visits per week with a client caseload of up to 40 case management clients a month, weekly reports, documentation, data collection, file maintenance, coaching, mentoring, and social service support to the client.

Position Summary:

Under general supervision this position will perform differential response services to clients in accordance with the funding contract. This may include, but is not limited to needs assessments, referrals to community resources, one-on-one coaching, CFT coordination/participation, follow-up contact, unannounced home visits, weekly reports, documentation, data collection, file maintenance, coaching, mentoring, and social service support to the client. The Safety Net contract serves a high number of 4 types of referrals, Differential Response, Case Management, Concrete Support and Post Reunification.



GENERAL REQUIREMENTS:

Program Specific Duties:

1.    Provide intense home-based services to assigned families; focusing on development of family needs and service plans, access to various resources, and overall support with mental health issues challenging the family and/or identified client.

2.    Complete home visits independently to meet with clients and families, must be able to utilize appropriate field safety judgement.

3.    Conduct risk assessments and follow safety protocols.

4.    Complete child abuse reports consistent with mandated reporting laws.

5.    Assume responsibility for case assessments, service plans, and reports for each case.

6.    Maintain Safety Net files and data base programs/trackers on a daily basis and assisting with any related statistical and/or evaluation components.

7.    Promote and maintain a sense of teamwork between Safety Net and other CAPC staff, CPS, community partner agencies, and other individuals relevant to the successful case management of each case.

8.    Assist the Program Manager as requested.

9.    Network with other community agencies and participate when requested, in community and health fairs and other events designed to promote CAPC programs.

10.    Establish trust with families through family centered support and encouragement.  Use strength-based approach and perspective at all times, while implementing motivational interviewing and knowledge of protective factors in work.

11.    Teach problem solving skills to enhance family functioning and assist in setting and achieving family goals.

12.    Attend IEPs, 504 plans, and SST meetings when appropriate and empower families to self-advocate when needed.

13.    Coach parents to act as advocates for themselves and their children to provide healthy/safe environment for children.

14.    Follow all contract and policy guidelines in regards visit timelines and paperwork.


Minimum Qualifications 

 

Education: 60- completed semester units with major coursework in psychology, sociology or related field REQUIRED. Graduation from an accredited four-year college or university required, with major course in social services or a closely related field preferred. 

 

Experience:  Experience working with children and families or social work case management.  Experience and enthusiasm for working with and supporting families; especially parents/caregivers.


Knowledge of:  Child welfare issues, Childhood development, and child abuse issues. Knowledge of community-based and social networks and resources.
