SAAF Mission

The San Antonio AIDS Foundation seeks to reduce HIV transmission, provide support and treatment for those living with HIV, reduce stigma and support positive sexual health. SAAF provides sexual health education, STI and HIV testing and counseling, medical care, case management, housing and meal support, mental health services and referrals for dental health. Access to care and early intervention have a significant impact on the health and well-being of people living with HIV. 


Position                                                                                                                                  

The Case Manager ensures that her/his assigned clients receive a coordinated range of client-centered services linking clients and family members with health care, psycho-social and other services through the assistance of Unit staff.

This position demands a high level of cultural competency of the unique needs of the LGBTQIA++ community, communities of color, and other marginalized populations.


Duties and responsibilities

  • Responsibility for own clients/cases assigned by supervisor
  • Assisting and encouraging clients to access medical care
  • Ensure that all eligible funding sources are tapped before accessing Ryan White funds.
  • Enter units of service and client data on a timely basis into the ARIES and Client Track Systems.
  • Maintain assigned clients medical and service record files and responsible for files completeness
  • Initial comprehensive assessment of clients' needs and support systems.
  • Development of a comprehensive, individualized service plan and coordination of the services required for implementation of plans for clients
  • Monitoring to assess the efficacy of the plan and periodic re-evaluation and revision of the plan as necessary.
  • Provide case management services to own panel of clients including:
  • Assist clients in accessing routine medical care for HIV.
  • Assist clients in managing their debts including Care Link Program with University Hospital System.
  • Assist clients in applying for Social Security benefits and other types of entitlement programs.
  • Refer appropriately to other AIDS and social service agencies.
  • HIV education and prevention
  • Client's compliance with medications; education as necessary
  • Abide by current Ryan White Standards of Care
  • Perform other duties as assigned

Requirements

Qualifications

Bachelor's (Required) or Master's (Preferred) degree in Social Work, Nursing or related field. Several years’ experience in providing support services in a social service agency preferred. Experience providing HIV/AIDS-related social services, preferred. Ability to speak Spanish preferred, but not required. If waiver was historically granted by the Ryan White Administrative Agency, multiple years' experience in HIV may substitute for degree.

CONTINUING EDUCATION: The employee is required to maintain and expand his/her knowledge of HIV/AIDS, treatments and current requirements and eligibility for service entitlement programs. A minimum of 24 hours of continuing education in HIV/AIDS services or other job-related issues is required annually and any other requirements that their licensing board may require.


Working conditions

This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. The position also requires working with the community residents and difficult situations. Evening and/or weekend work may be required as job duties demand. 


Physical requirements

While performing the duties of this job, the employee is occasionally required to stand; walk; sit. The employee must occasionally lift or move up to 25 pounds. 

As SAAF works daily with clients who may be immunocompromised, evidence of COVID-19 vaccination is required.
