CHC is seeking licensed Family Nurse Practitioners to join our team! The Advance Practice Practitioner - Family joins a collegial environment that promotes exceptional care for patients, teamwork, quality improvement and innovation. This role includes possible evening and weekend shifts. 


Basic Qualifications

Education 

  • Graduation from program of study accredited by the appropriate professional board
  • Board certification or eligibility for board certification.
  • Licensure for practice in Vermont.

  • Completion of requirements for successful CHC credentialing.

  • Ability to work effectively with a variety of ethnic, political, and socioeconomic groups and individuals.

  • Ability to work independently as appropriate to level of training and experience, and to solve problems creatively and with initiative.

  • Willingness to work flexible hours and flexible locations.

  • Commitment to supporting the mission and principles of the Community Health Centers.

  • Must be CPR certified.

Knowledge, Skills, and Abilities

  • Ability to work independently as appropriate to level of training/experience
  • Ability to work collaboratively with multiple disciplines to support excellent patient care
  • Ability to problem solve and collaborate in the care of individuals complicated by insecure housing and other significant social determinants of health and mental health
  • Ability to solve problems creatively and with initiative
  • Ability to work flexible hours

Licensure, Certification, or Other Requirements

  • Board certified or eligible for board certification
  • For Physician Assistants, must have CAQ in Psychiatry
  • CPR certification
  • Licensure for practice in the State of Vermont
  • Completion of requirements for CHC practitioner credentialing


Compensation commensurate with direct experience within the range of $109,946- $130,394 base pay annually.
