City Care is searching for the right person with a knack for seeing great potential in others and working as part of the team with the shared goal of motivating change toward that potential. We believe that our best advocate role is lived out by being in relationship with those that are working hard to believe in a bright future for themselves.

We are looking for an organized and task-oriented Advocate who is well equipped to work with single adults on a flexible schedule. The City Care Housing Navigator’s primary role is to assist individuals within the homelessness response system transition into safe and affordable housing by helping them meet their goals and objectives within a psychosocial rehabilitation framework.

Responsibilities include but are not limited to:

  • Carry out the mission, vision and values of City Care.
  • Well-versed in community resources, developing a network of collaborative partners
  • Develop housing individualized plans with guests based on their self-directed goals
  • Improve guests’ outcomes by collaborating to develop, monitor, and evaluate service plans
  • Provide case management assistance, resource development, crisis intervention and direct
  • Support to individuals in the shelter (including document recovery, collaboration with the City of
  • Norman’s Continuum of Care and other partner agencies to leverage resources and facilitate
  • progress)
  • Promote housing stability for each resident by documenting progress, referrals and contacts in the electronic record. Ensure ROI’s, case notes and weekly documentation is updated and in compliance with best practices.
  • Educate new incoming guests about rules and policy.
  • Engage in weekly face-to-face meetings with guests as directed
  • Attend weekly staffing and community meetings as directed
  • Uphold professional confidentiality among guests, interns, externs, volunteers and staff.


Requirements:

  • Previous experience in case management, recovery, or mental health services.
  • Computer literate, resulting in error-free data entry
  • Must have a current Oklahoma driver’s license, reliable transportation and updated car insurance.


Preferred:

  • Professional experience navigating mental and physical health systems, benefits and resources
  • Previous shelter experience
  • Behavioral Health Case Manager II; Associates Degree, or equivalent lived or work experience
  • HMIS experience


Full Time 

Salary Range: $19.50 hr.

Flexible schedule consisting of some days and mostly evenings,  with every 7th week off for PAID REST WEEK for Full-time employees.
