About the Role:

The Facility Rehab Lead plays a critical role as  the on-site representative of UT Health at Home within our partnered congregate living communitites. This role ensures seamless coordination of clinical and non-clinical servies to promote wellness, safety, and a superior resident experience.  

Assist with scheduling and requiing skilled nursing and therapy visits for residents. 

Attend and contribute to daily stand-upmeetins by deliverin and receiving reports on home health patients.

Lead and manage non-clinical wellness services 

Education:  High school graduate

Experience: Six months to noe-year related experience required

Skills:  Good interpersonal and organizational skills.  Knowledge of medical terminology preferred.
