Annandale Village is a one-of-a-kind nonprofit community in Suwanee, GA, where we proudly serve adults with developmental disability and acquired brain injuries. We are a true community where our residents, known as Villagers, are supported in living with dignity, purpose, and independence.


We’re the only nonprofit in the southeast offering a full continuum of care for the people we serve. Our 55-acre campus includes: 

  • Two assisted living buildings serving 33 Villagers
  • Eight Personal Care Homes (independent, semi-independent, and cottages) serving up to 100 Villagers
  • A 32-bed skilled nursing facility—the only one in Georgia dedicated solely to adults with developmental disabilities
  • A Medicaid waiver day program with approximately 33 participants
  • A community-based independent living program, All In, currently serving 26 individuals

Position Purpose

Responsible for planning, organizing, directing and implementing Villager programs and services through the Personal Care Home (PCH) Residential Services and Social Services in accordance with applicable regulations, as well as direct service provision in PCH/residential programs during nights, overnights and weekends.

Required Qualifications

Bachelor’s degree in behavioral science, Education, Social Work or Psychology required with a master’s degree preferred.

Proven knowledge and skills in personnel, supervision, management, program design and presentation.

Developmental Disabilities/Mental Health and/or Personal Care Home experience. 

Knowledgeable of developmental/intellectual disabilities or acquired brain injuries. 

Personal Care Home Administrator license preferred. At a minimum must meet requirements for eligibility and obtain within the first 90 days of employment.

Must be at least 21 years of age, have a valid Class C Georgia Driver’s License, proof of vehicle insurance, and meet company policy regarding MVR requirements. 

Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.   

Must undergo and pass a drug screening and screening for Tuberculosis. 


Major Duties and Responsibilities

Manages and oversees the planning, budgeting, and implementation of all Villager programs and services, under the Personal Care Home and Social Services Programs.

Ensures all Villager programs and services are in compliance with Annandale’s policies and procedures, state licensing requirements, and the rights of all Villagers through consistent and ongoing review, development, and implementation of policies and procedures.

Responsible for ensuring positive and productive communication between departmental staff, management team, and administrative team.

Ensures Annandale’s program quality and person-centered focus meets the individual developmental abilities of each Villager and addresses the individual desires expressed in his/her ISP.

Ensures family input, involvement, and support for Villager services.  Facilitates the resolution of family issues or concerns in conjunction with the departmental managers.

Works with the Financial Services department to develop, plan, and implement annual budgets for the Programs and Services departments.

Oversees and coordinates personnel management and departmental staffing profiles along with the two departmental managers or supervisors (Personal Care Home Supervisor and Social Services Manager). 

Supervise and provide leadership to the Personal Care Home Nurse Managers, Medication Technicians, Personal Care Home Coordinator, and Personal Care Home Professionals. 

Responsibilities include coordinating daily operations, ensuring adherence to regulatory standards, facilitating staff development, addressing performance issues, and fostering a collaborative environment to deliver exceptional resident care.

Prepares correspondence for families, Executive Committee, Board of Trustees, and others as required.

Works with departmental leaders to plan and coordinate daily operations to ensure a seamless integration and delivery of programs and services to the Villagers.

Works on site directly and/or through supervision or management in the Personal Care Home setting at least once/week and across all shifts (day, evening, night, weekend).

Ensures that the Villager’s participation in local clubs, organizations, and church groups is appropriate and meaningful.

Ensures that the Social Services Manager and other relevant staff are adequately trained and informed on all Medicaid issues.

Ensures the safety and security of Annandale staff and Villagers through training and implementation of security policies and procedures with the assistance of the Compliance Director and Shift Coordinators.

Through the Departmental Managers, design and implement Villager learning and training opportunities, Villager life experiences, and client centered, individual supports and services.

Reviews and monitors all incident reports and provides feedback to the Compliance Director as needed.  

Responsible for ensuring critical incident reporting to the appropriate state agencies.

Monitors and supervises all audits of Villager’s records to ensure state, division, and Annandale requirements are met.

Acts as the contact person for Personal Care Home audits.   

Coordinates the team to assist with the audits and facilitates any corrective action plans necessary. 

Oversees the Admissions Committee for the Personal Care Home Program, reviewing all potential Villager applications to ensure appropriateness for placement, including residence, completion of file contents, and coordinates with the Admissions Director, and Social Services Manager in preparation for admission/placement.

Ensures that the milestone reviews for all Villagers support and service plans are scheduled and implemented with the Villager’s family/guardian in a timely manner and that the reviews are accurate and meaningful.

Performs other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care needs of the facility residents.

vEmployee is required to transport (drive) and assist Villagers in and out of vehicles.

vEmployee is required to perform CPR/First Aid.
