Summary: The Thriving Families Case Manager plays a vital role in supporting families with school-age children who are at risk of housing and financial instability. This position focuses on preventing homelessness, fostering family stability, and promoting educational continuity for children. Through a holistic, trauma-informed, and two-generation approach, the Case Manager works with both parents/caregivers and children to assess needs, set goals, and connect to community resources. This role requires a balance of compassion, strategic thinking, and strong organizational skills to deliver high-quality case management and achieve lasting outcomes for families.
Success In This Role Looks Like:
- Building trust-based, goal-oriented relationships with families to support stability and long-term success.
- Managing a large, diverse caseload while maintaining high-quality service delivery and accurate documentation.
- Coordinating with internal programs and community partners to provide wraparound support in areas such as housing, employment, education, healthcare, and financial coaching.
- Advocating for families to address barriers to housing stability and children’s school success.
- Tracking and reporting outcomes in compliance with program standards and funding requirements.
What You Bring To The Team:
- Experience in accurate, timely data entry and use of case management systems.
- Cultural humility and the ability to equitably serve families from diverse backgrounds.
- Strong organizational skills, with the ability to manage multiple priorities independently in a fast-paced environment.
- A Master’s degree in Social Work (MSW) from a CSWE-accredited school is preferred.
- Alternatively, a Bachelor’s degree in Social Work (BSW) or an undergraduate degree in Human Sciences (Sociology, Psychology, Counseling, etc.) and a minimum of 5 years of trauma-informed human services case management experience is required.
