Job Summary:
The Foster Care Patient Navigator serves as a liaison between foster care youth, caregivers, healthcare providers, the Children’s Division and Division of Youth Services. This role is responsible for coordinating care, removing barriers to services, and ensuring that youth in foster care receive timely, comprehensive, and compassionate health services with a trauma-informed approach. The patient navigator supports development, coordination, and monitoring of Jordan Valley’s foster care service program.
Key Responsibilities:
- Coordinate and schedule appointments for foster children, including physicals, dental exams, behavioral health, and specialty services as needed.
- Serve as a central point of contact for foster families, caseworkers, clinic staff, and external partners.
- Assist with gathering and verifying documentation (e.g., consent forms, custody paperwork, insurance details, Medicaid eligibility).
- Educate caregivers and patients about available services, care plans, and follow-up needs.
- Document patient encounters and case notes accurately and in a timely manner.
- Collaborate with internal departments (e.g., billing, dental, behavioral health) to streamline care.
- Ensure patient information is protected and handled in accordance with HIPAA and foster care confidentiality regulations.
- Assist with patient transportation coordination when needed.
- Track and report data on foster care patient volumes, service completion rates, and other quality metrics.
- Track patient appointments, follow-ups, and compliance with foster care medical requirements.
- Input and maintain accurate records in the electronic health record (EHR) and foster care tracking tools.
- Compile and analyze program data for reporting and quality improvement.
- Coordinate with clinic staff, billing, dental, behavioral health, and external partners such as Children’s Division or foster agencies.
- Assist with training as needed.
- Help prepare documentation for audits, grants, or internal reviews.
- Participate in team meetings, case staffing, and foster care program planning.
- Connect patients with resources to meet basic needs and to improve the patient’s social determinants of health.
- Will need to complete a certified Community Health Worker Class and maintain a CHW Certification within 1 year of employment.
- All other duties as assigned.

Requirements
- High school diploma or equivalent (required); associate or bachelor's degree in social work, Health Sciences, Public Health or related field preferred.
- 1-2 years of experience in case management, health care navigation, social services, or related role.
- Prior experience working with foster care populations or vulnerable youth strongly preferred.
- Strong customer service and communication skills with the ability to assist diverse populations.
- Ability to multitask, work effectively in a fast-paced environment, and adapt to workflow changes.
- Proven ability to work collaboratively as part of a team and meet defined productivity goals.
