Description

The Designated Coordinator is responsible for ensuring that all 245D Home and Community-Based Services are coordinated and delivered in compliance with Minnesota 245D Statutes and reflect person-centered, positive support practices. Designated Coordinators oversee the creation and implementation of each individual’s Coordinated Services and Supports Plan (CSSP) Addendum, which includes various documents and ensures the health, safety, and rights of each person served.

Minimum Qualifications (per MN Statute 245D.081)

  • Bachelor’s degree + 1 year full-time direct care experience, OR
  • Associate degree + 2 years full-time direct care experience, OR
  • Human services diploma + 3 years full-time direct care experience, OR
  • 4 years full-time direct care experience and 50 hours of education and training related to human services and disabilities.
  • Valid NetStudy 2.0 background study clearance.
  • First Aid certification or annual basic first aid training
  • Rule 9544 and DHS required trainings completed when due
  • Strong written and verbal communication skills, documentation accuracy, and organizational skills
  • Ability to work independently while maintaining compliance with all 245D requirements

Requirements

Key Responsibilities

  • Coordinate and document implementation of each individual’s Coordinated Service and Support Plan Addendum
  • Coordinate with case managers, lead agencies, healthcare professionals, and legal guardians
  • Develop person-centered support plans and ensure that Positive Support principles and practices are used
  • Monitor and document service delivery for compliance with MN 245D Statutes
  • Participate in the admission, service initiation, and service delivery process
  • Home visits with the individuals served, when required by 245D regulations, the support plan or the support team
  • Train, support, and oversee direct support staff and perform competency checks and yearly evaluations
  • Facilitate progress on measurable outcomes and evaluate the effectiveness of methodologies used
  • Report maltreatment, incidents, and injuries per established regulatory timelines and conduct internal reviews
  • Complete and maintain accurate, timely documentation and progress reviews
  • Protect and promote the rights of each person served
  • Follow all applicable agency policies and DHS regulations
  • Complete all required annual trainings when due
  • Cooperate with internal audits and DHS reviews and ensure implementation of corrective action plans if identified 
  • Adhere to all requirements and remain current with changes to MN Statutes 245D, the Positive Support Rule (MN Rule 9544), and related DHS regulations
  • Act with integrity, confidentiality, and professionalism in all duties
  • Provide direct care as needed
