Jacksonville Orthopaedic Institute is looking for someone to work PRN as a Physical Therapist at the San Marco Rehab.


The Physical Therapist provides an evaluation and assessment, design a written plan of care, implementation and supervision of treatment programs and educational services.

Office Location: San Marco Rehab, 1325 San Marco Blvd, Jacksonville, FL 32207


Schedule: Monday - Friday- as needed.


Essential Duties and Responsibilities 

  • Evaluation and assessment of patients referred to the Center.
  • Formulation of specific plans of care and goals for each patient.
  • With physician approval, implementation of the physical therapy services defined in the patient care plans.
  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
  • Identify and document goals, anticipated progress, and plans for reevaluation.
  • Record prognosis, treatment, response, and progress in patient's chart / EMR system.
  • Supervise, as needed, the administration of physical therapy services by physical therapy assistants and certified athletic trainers. 
  • Adhere to the established policies and procedures of the Center.
  • Participate in plan of care reviews, patient case review conferences, patient assessment and reassessments.
  • Ensure completion of all required documentation of patient care activities.
  • Provide necessary education services of the patient and family members when appropriate.
  • Assist the Site Coordinator as directed with, but not limited to: Utilization Review, Quality Assurance, Program Evaluation, Infection Control, and Safety Committee activities.
  • Participates in center in-service activities.
  • Assist in maintaining a clean facility, including physical plant and equipment maintenance and supply inventory upkeep.
  • Supervise physical therapy or athletic training students as directed by the Clinical Site Coordinator.
  • Adhere to HIPAA policies and procedures.

 

Position Requirements

 

  • Must be duly licensed under applicable state laws to practice physical therapy.   
  • Graduate of an accredited physical therapy curriculum.  
  • Successful completion of Basic Cardiac Life Support. 
  • Current CPR card.
  • Demonstrated ability to carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
  • Demonstrated ability to deliver quality, effective patient care.
  • Ability to perform duties in an efficient and independent manner with minimal supervision.
  • Knowledge of federal, state, and local legal and professional requirements.
  • Effective communication skills both verbally and in writing.
  • Computer proficiency, including experience with medical software and EMR.

Fast paced orthopedic medical practice environment.  

 

Physical Demands

 

Must be able to stand for long periods of time.  Must be able to lift, bend, walk, kneel, stretch, and stoop. Requires eye hand coordination and manual dexterity.   Requires normal hearing and eyesight to allow for patient interaction and processing of patient information.    Ability to move/lift 10-15 pounds frequently and up to 40 pounds occasionally.
