Rogue Community Health is looking for a RN Nurse Case Manager in our School Based Health Center locations. School Based Health Center who is not only committed to exceptional patient care, but equally interested in supporting staff within a cohesive team environment. 

 

This position will ensure the efficient and effective flow of students/patients during scheduled school hours. Within the RN scope of practice, utilize the nursing process to provide patient care and education which will focus on patient engagement and behavioral change, treatment compliance, and health risk factor reduction. A School-Based Health Center is a unique health care model for comprehensive physical, mental, and preventive health care services for children and adolescents in a school setting. All registered students and school staff are eligible for appointments provided there is a consent form on file.

This position works full time hours during the school year only with two weeks off at Christmas and 9 weeks off during summer vacation or one can be full time and cover the main clinics when school is not in session. This position does have an annualized salary based on the school year. 

 

EDUCATION: Graduation from an accredited school of nursing; BSN preferred.
