The Vice President Post-Acute is a key member of the Franciscan senior executive team serving as a catalyst for aligning clinical goals and business objectives. In this role you will oversee and provide system level leadership for the operations and clinical care delivery for Home health, Hospice, and the Program for All Inclusive Care of the Elderly (PACE). This role is accountable for setting the overall direction, strategy development and operational achievement for the organization plan for the delivery of quality, compassionate, cost-effective, and efficient post-acute care to individuals, families, groups and communities.

WHAT YOU CAN EXPECT
This position will be located out of any of our Northern, Central, or Western Indiana locations within closest proximity.
Be an advocate for the best quality of patient care; ensure post-acute quality and safety metrics are achieved.
Represent post-acute care at provider executive committees and other provider staff committees.
Collaborate with provider leadership to develop health care protocols, policies, and procedures for ensuring quality care delivery.
Design continuum of care options for managing patient throughput including acute care settings, long term and skilled nursing care units, urgent care centers, and admission/discharge units. Focus on improving specific areas of patient care by analyzing the root causes for variation from quality standards, improving the process, controlling solutions and sustaining success.
Facilitate and reinforce a mindset for excellence to reduce unnecessary variations, improve clinical outcomes and reduce costs across the continuum of care.
Initiate and sponsor the development and implementation of quality and performance improvement projects.
Meet with the members of the community, physician leaders, healthcare organizations or federal/state/regional officials on matters of mutual concern and to address community needs.
Oversee the effective delivery of service to those we are privileged to serve, evaluating satisfaction, and to ensure the organization is meeting quality service standards.
Serve as the Senior Leader for Post-Acute Care, ensuring that the clinical and patient perspective is included in organizational decisions.
Establish goals and objectives for the Post-Acute service line consistent with and in support of the strategic and operational objectives and the mission, vision and values of the organization.
QUALIFICATIONS
Bachelor's Degree Nursing or Healthcare Administration (Business) - Required -AND-
Master's Degree Nursing or Healthcare Administration (Business) -Required -OR -
Doctorate Physical Therapy - Required - AND -
10+ years Progressive leadership experience within an integrated healthcare delivery system or large post-acute deliver network with recent track record of demonstrated outcomes. - Required
3 to 5 years Administrative Director or VP level experience in Post Acute that includes Home Health, Hospice, and preferably Program for All Inclusive Care of the Elderly (PACE), home infusion and durable medical-equipment program - Required
Registered Nurse (RN) - State Licensing Board - Required -OR-
Physical Therapist (PT) - State Licensing Board - Required
