Children's Pharmacy has an opportunity for a Medication Reconciliation Specialist!
Work schedule: Full Time and Hours are rotating 1st and 2nd shift 0800-1630 and 1400-2230 M-F
Some holidays will be required when the holiday falls on a M-F
Location: Remote but must be local to MKE to come in as needed
What you will do:
Under general supervision and according to established procedures, the Medication Reconciliation Specialist will assist in the medication reconciliation process. The Specialist will conduct initial patient, family, or primary caregiver interviews to collect a complete and accurate list of all prescription, over the counter, and alternative therapies the patient was taking prior to hospital admission. The accuracy and completeness of this medication information is critical to the continuity of care and positive patient experience. The outcome of this interview is a timely, complete, and accurate medication list that is available for pharmacist review and clinical patient decision making.
High school graduate or GED
Previous healthcare experience required; Pharmacy technician experience (hospital or retail) or medication reconciliation experience preferred
Previous experience must include a minimum of at least six months in a role where the individual interacted with medications and/or the electronic medication record (i.e. has a pharmacy technician or medical assistant)
Registered as a pharmacy technician in the state of Wisconsin or able to become registered within 30 days of hire.
Strong customer service skills.
Verbal expression is clear, positive, and appropriate.
Effectively communicate to the appropriate audience and alters communication style according to the audience demonstrating situational adaptability.
High level of attention to detail, accuracy, and critical thinking when interpreting these details.
Strong sense of curiosity and desire to research and problem-solve based on self-identified questions.Ability to identify gaps in available details and seek further information when necessary.
Ability to draw reasonable conclusions from a set of information and discriminate between useful and less useful details to solve a problem and make decisions that will impact patient care.
Basic typing and computer skills.
Strong proficiency in mathematical calculations such as addition, subtraction, multiplication, division, fractions and proportions at a level normally acquired through completion of high school.
Previous electronic health record experience required, and experience with the EPIC platform is strongly preferred.
