The Patient Care Unit Coordinator assists the RN Clinical Coordinators in providing direction and clinical support for a specified unit and assigned staff. This position performs administrative support activities required for proper transcription of medication, treatment orders, documenting and coordinating patient care. The Patient Care Unit Coordinator will maintain clinical records of the hospice patients and will function as a liaison between other team members assisting with day-to-day management duties.

Reports to the Director of Pediatrics & Specialized Services, RN Clinical Coordinators, and/or the Director of Nursing


FLSA Status: Non-Exempt

Requirements

Essential Job Functions:

  1. Fosters and maintains an effective working relationship between co-workers, patients, and other professional members of the community
  2. Assures patient records are complete and all required documentation is filed in a timely manner
  3. Enters patient data and visits into the EMR system
  4. Orders and monitors supplies and medical durable equipment
  5. Manages transport for LOC changes
  6. Responsible for faxing paperwork required by facilities
  7. Assists with scheduling and staffing as requested
  8. Assists in submitting claims for authorization and checking Medicaid eligibility monthly
  9. Assures patient records are complete and documentation is filed in a timely manner
  10. Prepares documentation for interdisciplinary team meetings as directed
  11. Accepts field and on call assignments are needed
  12. Fills in on the switchboard for lunches, absences and time off as required
  13. Other duties as assigned

Physical/Safety Requirements:

  1. Must have the strength and endurance required for standing, walking, lifting, bending, and pushing on a consistent basis up to 20 lbs
  2. Utilizes appropriate durable medical equipment as needed in a safe manner
  3. Uses appropriate body mechanics as needed for safe patient care
  4. Persons in this position may be subject to potential exposure of infectious waste, disease, and other conditions, including TB, HIV and Hepatitis B
  5. Utilizes appropriate PPE as needed
  6. Follows the designated plan of action in the event of a fire or other emergency/disaster
  7. While performing the duties of this job, the employee is regularly exposed to fumes or airborne particles, work-related stresses, and driving hazards
  8. The individual in this position may be required to drive to meetings with clients or health care professionals, as well as patient’s homes
  9. The noise level in the work environment is usually minimal

Qualifications and Experience:

  1. Two years’ experience as a Licensed Practical Nurse, Medical Assistant or Certified Nursing Assistant in hospice, home health or geriatric care preferred
  2. One year of experience in an office setting, medical office experience required
  3. Demonstrated expertise with Microsoft Office Suite applications and have general computer knowledge
  4. Excellent interpersonal and communication skills
  5. Must be able to work with a variety of personalities
  6. Successful completion of a background check pursuant to NRS 449.188 and NRS 179A.100 along with a pre-employment drug screen and physical, TB testing, previous employment and reference checking, and an MVR (may include verification of education)
  7. CPR/First Aid certified
  8. Must be able to work as a team
  9. Ability to function with limited supervision and accountability to meet assigned projects and timelines
  10. Must be able to handle frequent interruptions
  11. Must be able to cope with emotionally charged situations encompassing, patients, families, and other caregivers
  12. Must have reliable transportation, a valid driver's license and auto insurance
