Essential Duties and Responsibilities:
- Assure comprehensive healthcare needs are identified and treated for age-specific populations in collaboration with the supervising physician. These duties are to be performed within the Nurse Practitioner’s scope of practice and in accordance with PCHG’s policies.
- Provide education and give health management information and illness prevention techniques for the promotion of community health.
- Communicate effectively with all members of the Care Team to ensure smooth handoffs and a positive patient experience.
- Consult and coordinate with health care providers/clinicians working at any location.
- Mentor and train new providers or clinical support team members as needed.
- Work in coordination with the Quality Assurance Team and management to develop and implement a quality assurance program.
- Documents in a timely fashion accurate accounts of interactions with patients in Electronic Health Records in compliance with PCHG protocol.
- Communicates any concerns about patient care issues or interpersonal difficulties. Follows proper chain of command and proper channels of communication.
- Participate in a rotation for afterhours services on-call.
- Manage daily and weekly schedule to ensure encounter targets are met and documented appropriately in the Electronic Health Record (EHR).
- Review and address tasks daily and maximize productivity.
- Ensure confidentiality and HIPAA compliance at all times.
- Keep other care team members informed when situations occur that disrupts timely patient flow through site.
- Contribute to community give back initiatives and be an advocate for Premier’s mission.
- Adhere to patient care needs and the core values of Premier Community HealthCare to a provide best-in-class patient experience.
- Work towards attaining department goals and drive Quality Improvement / Quality Assurance (QI/QA).

Qualifications:
- Master's degree from an accredited Nurse Practitioner Program with a clinical specialty area of Family Medicine or Pediatrics.
- Current APRN license in Florida.
- Current Drug Enforcement Agency (DEA) License strongly preferred or ability to obtain.
- Current Basic Life Support (BLS) certification.
- Minimum of three (3) years’ experience as a Nurse Practitioner preferred.
- Obtain and maintain Epic certification/ annual compliance training throughout employment.
