Astria Health is seeking a Physical Therapist Assistant to join Home Health our team! This will support our Toppenish Hospital. 

The shift for this will either be 5 days 8 hours, or 4 days 10 hours. Schedule to be discussed at interivew/hiring process.

What you get to do every day!

As the Physical Therapist Assistant responsible for providing treatment interventions under the direction of a Physical Therapist based on the patient’s established goals and plan of care. You will:

• Utilizes appropriate therapy interventions to achieve the established goals set by the Physical Therapist.
• Assists with gathering the applicable data necessary for accurate documentation of services provided and achievement of treatment plan and goals.
• Utilizes appropriate therapy interventions to achieve established goals.
• Coordinates all services provided with treating Physical Therapist to assure services based on best practices.
• Educates patient, family and designated caregiver as appropriate to treatment plan.
• Performs other duties as assigned
• Collaborate effectively across organizational boundaries.
• Adheres to the core values and standards of the organization.
