About the Role:
The Case Management RN plays a pivotal role in coordinating and delivering comprehensive healthcare services to patients by assessing their needs, planning appropriate care, and facilitating access to necessary resources. This position ensures that patients receive high-quality, cost-effective care while promoting optimal health outcomes and patient satisfaction. The RN collaborates closely with multidisciplinary teams, including physicians, social workers, and other healthcare professionals, to develop individualized care plans tailored to each patient's unique circumstances. Additionally, the role involves continuous monitoring and evaluation of patient progress, adjusting care plans as needed to address changing health conditions. Ultimately, the Case Management RN serves as a patient advocate, guiding individuals through complex healthcare systems to achieve seamless and effective care transitions.
Minimum Qualifications:
- Current and valid Registered Nurse (RN) license in Nebraska.
- Associates Degree in Nursing required.
- Strong knowledge of healthcare systems, insurance processes, and community resources.
- Proficiency in electronic medical records (EMR).
- BLS required upon hire.
Preferred Qualifications:
- Minimum of three years clinical nursing experience,
- One year preferably in case management or related healthcare settings.
- Accredited Case Manager (ACM) credential.
- Experience working with diverse patient populations and complex chronic conditions.
- Advanced training in care coordination, utilization review, or patient advocacy.
- Familiarity with regulatory requirements such as HIPAA, Medicare, and Medicaid guidelines.
- Excellent interpersonal and communication skills demonstrated in multidisciplinary team environments.
Responsibilities:
- Conduct comprehensive assessments of patients' medical, psychosocial, and functional needs to develop individualized care plans.
- Coordinate and communicate with healthcare providers, patients, and families to ensure timely and appropriate delivery of services.
- Monitor patient progress and outcomes, adjusting care plans as necessary to meet evolving health needs.
- Facilitate access to community resources, support services, and healthcare programs to promote patient well-being.
- Document all case management activities accurately and maintain compliance with regulatory and organizational standards.
- Educate patients and families about disease management, treatment options, and available resources to empower informed decision-making.
- Participate in interdisciplinary team meetings to discuss patient care plans and collaborate on problem-solving.
- Ensure adherence to ethical standards and confidentiality requirements in all aspects of case management.
