Position Summary:

- Support value-based care initiatives by focusing on quality outcomes, reducing avoidable hospital readmissions, optimizing cost of care, and using data to guide clinical decisions and improve performance.
- Collaborate with physicians and interdisciplinary team members to ensure coordinated and efficient care, participating in care plan meetings and quality improvement efforts.
- Facilitate smooth transitions of care across different settings, ensuring continuity and minimizing gaps in treatment.
- Educate residents and their families about diagnoses, treatment plans, medications, and preventative care strategies to promote engagement and better health outcomes.
- Maintain accurate, timely, and compliant documentation adhering to all clinical, regulatory, and organizational standards.
Requirements
- Master’s degree from an accredited Physician Assistant program.
- Current and unrestricted Physician Assistant license in the respective state.
- Certification by the National Commission on Certification of Physician Assistants (NCCPA).
