Schedule: Full-time evenings 2:45p-11:15p (40 hrs/wk), consistent schedule, every other weekend required, holiday shifts as scheduled/needed (24/7/365)
Compensation: $28.50-$34.50 per hour, commensurate with experience.

As an LPN you will:
- Support, guide, and mentor Resident Aides and Medication Technicians, fostering a team culture that delivers compassionate, consistent, high-quality care;
- Safely administer and monitor medications and treatments according to physician orders, facility policy, and DOH ACF/EALR standards—ensuring every resident receives the care they deserve;
- Observe and document changes in residents’ health, communicating promptly with the RN, physician, or appropriate provider to keep residents safe and supported;
- Assist in implementing and reviewing Individualized Service Plans (ISPs), helping each resident receive care tailored to their unique needs;
- Uphold DOH-approved Medication Management and Infection Control practices to protect residents, staff, and the community;
- Help residents with Activities of Daily Living (ADLs) while encouraging confidence, autonomy, and meaningful engagement;
- Inspire resident participation in social, recreational, and wellness programs that enhance joy, connection, and overall quality of life;
- Maintain accurate, thorough clinical documentation and ensure compliance with state and facility recordkeeping expectations;
- Participate actively in care planning, quality assurance efforts, and ongoing staff training;
- Honor resident rights, privacy, dignity, and individuality in every interaction;
- Contribute to maintaining a clean, supportive, and nurturing environment where residents can truly thrive.

Requirements
- At least 18 years of age;
- Hold a current New York State LPN license in good standing;
- One (1) year of relevant experience or equivalent combination of education, training, and experience in a medical or healthcare setting;
- Demonstrate strong leadership skills including good judgment, problem-solving, and decision-making capabilities;
- Experience working with older adults preferred;
- Able to push, pull, lift, and carry trays of food, laundry bags, heavy boxes, and/or equipment weighing up to 50 pounds without restrictions;
- Must be able to stand for long periods of time, frequently bending, stooping, and reaching;
- Satisfactory completion of post-offer, pre-employment physical required.
