Summary
The RN Visiting Care Coordinator is responsible for planning, coordinating, and ensuring continuity of patient care, while maintaining strong relationships with the patient, family, and physician from admission through discharge. Key responsibilities include patient advocacy, clinical competence, and effective resource management.
Schedule
- Fulltime position | 64 hours per two weeks
- Monday through Friday, 8:00AM to 4:30PM
- Includes on-call shifts, every 4th-5th weekend and holiday rotation
Pay and Benefits
Starting pay begins at $33.15 per hour and increases with experience
- Pay range: $33.15- $50.96per hour

Qualifications
- Current MN Registered Nurse License.
- Driver’s License required.
- Reliable transportation to make client home visits.
Department
The Community Based Patient Care Coordinator (PCC) uses a collaborative approach through which patient care is organized to support the best practice, desired patient outcomes and efficiency during a patient's episode of care.
