Role: Licensed Specialist Clinical Social Worker (LSCSW)
Department: Care Coordination / Care Management
Location/Type: Kansas • Hybrid (50% in-home, 50% telehealth)
Employment: Full-time, W2 with benefits
Pay: $55/hour
Schedule: Monday–Friday, 8–5
What you’ll do
Conduct KanCare health risk assessments (home, telehealth, phone)
Complete biopsychosocial, mental health, substance use, and SDOH screenings
Build person-centered plans of service with measurable goals
Submit HRAs and POS within 7 days
Collaborate with NPs, PCPs, and care managers
Document all findings in EMR/EHR on time
Join case reviews, huddles, and quality projects
Educate members and caregivers on benefits and resources
Must-haves
Active Kansas LSCSW license (BSRB)
Master’s degree in Social Work
2+ years in clinical social work/behavioral health/care management
Experience with Medicaid/KanCare populations
Proficiency with EMR/EHR documentation
Reliable transportation for in-home visits
