About Us:
Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain, and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized together with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. The network works together to create opportunities for older Ohioans to receive needed home and community services and supports; and to age successfully in their own homes and communities.
Position Overview: The LPN Community Resource Consultant assesses individuals’ Level of Care for initial eligibility or continued participation in the MyCare Ohio Waiver, Residential State Supplement, or PACE programs. Assessments can occur in their private residence, nursing facilities or hospitals.
Job Duties

Conduct all types of Level of Care assessments within the community alongside consumers (caregivers/POA/Guardians) to identify unmet needs, determine appropriate care levels, and assess any potential health or safety risks, all under the supervision of a Registered Nurse.

Collaborate and communicate with caregivers, healthcare and service providers, and other formal and informal support systems to accurately identify and address each individual’s needs.

Provide education to consumers, caregivers, and stakeholders regarding eligibility for the relevant programs.

Document all activities in the designated software systems in compliance with contractual requirements, internal policies, and quality standards.

Work constructively with internal WRAAA staff as well as external partners, both formal and informal.

Advocate for clients, especially in situations involving health or safety concerns, or when there is a risk of institutionalization.

Maintain at least minimum proficiency with all necessary computer applications and software programs.

Contribute to achieving divisional and agency-wide goals through active participation.

Ensure confidentiality of consumer and provider information in accordance with HIPAA and agency policies.

Promote positive employee relations and deliver excellent customer service.

Complete all required documentation accurately and within designated timeframes.

Participate in trainings to maintain and enhance job-related skills.

Carry out job responsibilities with respectful communication and professional behavior.

Engage in departmental and agency initiatives aimed at improving quality, efficiency, and stakeholder relations.

Perform assessments primarily during core business hours, with occasional early evening or weekend hours as needed to serve the community.

Assist and provide back-up to other department units as needed, and perform other duties as assigned.
Requirements

Active Licensed Practical Nurse (LPN) licensure in good standing with the Ohio Board of Nursing.

At least one year of experience in home care, geriatrics, home health care, behavioral health, long-term care, or an equivalent as approved by the Ohio Department of Aging.

Knowledge of major chronic illnesses, disease processes, normal aging, and medical terminology.

Strong verbal and written interviewing and assessment skills.

Excellent interpersonal and communication skills to represent the agency professionally and maintain effective working relationships within and outside the organization.

Intermediate proficiency in Microsoft Office and related computer applications including the ability and willingness to learn state-issued documentation platforms.

Ability to perform job duties remotely via telecommunication from both home and the field.

Capacity to document information accurately and promptly during interviews and meet all required timelines for program compliance.

Ability to carry and transport a laptop and case, with the physical ability to lift at least 20 pounds.

Capability to access private homes or locations that may not be easily accessible, requiring the ability to ambulate as needed.

Access to a car during working hours and the legal ability to drive.
