Responsibilities:
- Complete and submit MDS assessments on schedule.
- Work with the care team to make sure assessments reflect each resident’s needs.
- Help build and update care plans, keeping them individualized and relevant.
- Review documentation for accuracy and consistency.
- Keep track of deadlines and assessment schedules.
- Monitor quality measures and reimbursement systems tied to MDS.
- Join care plan meetings with residents and families.
- Support staff with training and guidance around documentation and compliance.
- Assist during audits or surveys.
Qualifications
- Active LVN or RN license in Texas (required).
- Minimum experience working in a Skilled Nursing Facility (required).
- Prior experience with MDS coordination preferred, but strong SNF background will be considered.
- Familiarity with RAI, care planning, and Medicare/Medicaid compliance.
- Highly organized with strong attention to detail; capable of prioritizing multiple tasks under tight deadlines.
- Comfort working with EHR systems, particularly PointClickCare (a plus).
- Strong interpersonal skills, able to effectively communicate with staff, residents, and families.
- Committed to high standards of compliance and quality in resident care.
