About the Role:
Family Circle of Care (FCC) is seeking a compassionate and skilled Two Licensed Vocational Nurses (LVN) to join our care team. This role provides direct patient care, supports providers, and helps ensure our clinics deliver high-quality, patient-centered healthcare.
What You’ll Do
Provide clinical care under the direction of RNs, Nurse Practitioners, and Physicians
Intake and exit patients, obtain vital signs, and document findings in EMR
Administer immunizations and medications as ordered
Collect and process lab samples (urine, blood, sputum, etc.)
Monitor patients for reactions to treatments/medications and report to provider
Assist with nursing telephone triage and patient follow-up calls
Maintain and order medical supplies, including vaccines and reporting to the State of Texas (VFC program)
Support Site Manager with daily operations, workflow coordination, and staff scheduling assistance
Educate patients on care plans, test results, and next steps
Participate in team huddles, quality improvement initiatives, and compliance programs
Minimum Qualifications:
Education/Experience: Associate’s degree or completion of Vocational Nursing program; 1+ year of clinical nursing experience preferred
License/Certifications: Current LVN license in Texas and active BCLS Healthcare Provider card (renewed every 2 years)
Preferred Qualifications:
Skills: Strong communication, patient service, and organizational abilities; EMR experience required (EPIC preferred); proficiency in Microsoft Word and Excel
Other:
Must be able to lift/move up to 50 lbs. and work in a clinical environment with exposure to infectious materials
