Cheer Home & Health Care, Inc. is hiring a Physical Therapist in San Diego. The Physical Therapist responsibilities will include:

Responsibilities:

• Provide evaluations, treatments, and education per Plan of Care
• Perform initial and ongoing patient assessments
• Develop and update treatment plans with care team
• Notify physicians of patient condition changes
• Document care notes promptly (within 24 hours)
• Establish therapy goals and deliver appropriate interventions
• Ensure patient safety, infection control, and proper equipment use
• Train patients/caregivers on therapy and equipment use
• Assess home environment safety and recommend solutions
• Maintain confidentiality and professional conduct

Requirements

• Active California PT license
• 3 years of experience

• Industry Experience, preferred
• Upbeat personality
• Strong Computer Skills
• Ability to multitask, drive, take action
• Strong Organizational & Communication Skills
• Pass Live Scan Fingerprint Clearance
• Valid driver’s license and proof of insurance required
• Cell phone with Internet Access
• Proof of Negative TB results
