Position: Full-Time Licensed Practical Nurse (LPN)
Location: Scottsdale/Phoenix
Shift: PRN/Per Diem as needed (weekdays)

Compensation and Benefits:
- Competitive pay per visit starting at $41/visit with a $30/ NVA

Qualifications:
- Graduate of an accredited practical nurse or vocational nursing program
- Minimum one (1) year of experience, preferably in home care or med/surg
- Current licensure in the state and CPR/BLS certification
- Strong communication and problem-solving skills
- Ability to meet physical demands of the job
- Valid driver's license and reliable, insured vehicle

Responsibilities:
- Implement personalized care plans under clinician supervision
- Maintain accurate and timely documentation
- Provide patient and family education on diagnoses and care plans
- Coordinate with interdisciplinary team members for comprehensive care
