About the Role:
Touchstone Health is seeking an experienced RN or LVN to serve as a Quality Assurance Coordinator across our Home Health and Hospice divisions in San Antonio, TX. This is an opportunity to work within a collaborative, clinically integrated team dedicated to exceptional patient care — while enjoying the flexibility and autonomy to shape your day.
Key Responsibilities:
- Oversee clinical documentation for accuracy, compliance, and timeliness in both home health and hospice
- Conduct OASIS review, Plan of Care audits, and identify documentation trends
- Serve as a liaison between QA, field staff, and clinical leadership to resolve discrepancies and improve performance
- Maintain up-to-date knowledge of Medicare Conditions of Participation and industry standards
- Provide coaching and education to clinicians on documentation best practices
- Participate in QAPI activities and assist in implementing quality improvement initiatives
- Support clinical teams during survey readiness, chart audits, and compliance reviews
- Collaborate with leadership on process improvements and clinical staff development
Requirements
What We’re Looking For:
- Registered Nurse (RN) (LVN) with current Texas licensure
- Minimum 2–3 years of QA experience in both home health and hospice settings
- Deep understanding of OASIS, Medicare guidelines, and regulatory compliance
- Strong attention to detail and commitment to accuracy
- Skilled in clinical documentation review and clinician support
- Excellent communication and collaboration skills
- Comfortable working closely with interdisciplinary teams
- Passion for quality, integrity, and patient-centered care
