We are seeking an MDS Registered Nurse to join our team and play a key role in assessing, monitoring, and documenting patient care while ensuring compliance with state and federal regulations. This position is essential in coordinating care, managing patient information, and facilitating communication between the healthcare team, residents, and their families.
Responsibilities:
- Conduct MDS assessments for residents with admission, quarterly, upon significant change, discharge, and tracking records.
- Collect and evaluate data related to residents' physical, psychological, and social well-being, including diagnoses, medications, treatments, and therapies.
- Use MDS data to identify concerns and develop personalized care plans.
- Collaborate with the interdisciplinary team (IDT)—including physicians, nurses, therapists, social workers, and nutritionists—to create and update care plans.
- Communicate with residents and families to incorporate their preferences and goals into care plans.
- Monitor the effectiveness of care plans and make adjustments as needed.
- Ensure accurate documentation to support appropriate reimbursement based on MDS assessments.
- properly assess, document, complete MDS, and create care plans for skilled residents in a timely manner.
- Participate in quality assurance and improvement initiatives to uphold high standards of care and compliance.
- Serve as a resource for staff on MDS assessments, care planning, and regulatory guidelines.
- provide training to nursing staff and care team members on MDS procedures and documentation requirements.
- maintain clear and effective communication with the care team and residents’ families.
- assigned on-call responsibilities.
Qualifications:
- Graduate of an accredited nursing program.
- Current Oklahoma RN license.
- Valid CPR and First Aid Certifications.
- Previous experience and knowledge of the MDS assessment process is required.
- Knowledge of federal and state regulations related to long-term care, Medicare, and Medicaid.
- Strong organizational skills with the ability to manage multiple priorities and maintain detailed records.
- Critical thinking skills to analyze resident data and develop effective care strategies.
- Ability to work collaboratively with the care team and lead care plan meetings.
- Prior nursing home experience is preferred.
