To provide nursing care in accordance with the client’s plan of care to include comprehensive health and psychosocial evaluation, monitoring of the client’s condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care.
Position Summary Expanded:
Under the Oklahoma Medical Marijuana and Patient Protection Act, (“The Unity Bill”) this position is considered a “Safety-sensitive position. “Safety-sensitive” is defined to include “any job that includes tasks or duties that could affect the safety and health of the employee performing the task or others.” Employees working in “safety-sensitive” roles are subject to this exception and subject to disciplinary action in the event of a positive test for marijuana or its metabolites.
Primary Position Responsibilities:
· Provides skilled nursing services in accordance with the orders of a physician and under the plan of care in patient’s home. Has the responsibility for the observation, assessment nursing diagnosis, counsel, care and health teaching of the ill, injured or confined and the maintenance of health and prevention of illness.
· Records the health needs of the patient.
· Performs and completes Comprehensive OASIS assessments and associated paperwork in accordance to agency policy to include start of care, recertifications, resumptions of care and discharge visits
· Coordinates the care of all patients in his/her case load
· Makes referrals to other agencies or services as needed
· Provides summary reports to the patient’s physician about patients under care, when the patient’s conditions changes, deviations from the plan of care or at least every 60 days.
· Participates in in-service programs, staff meetings and weekly Case Conference meetings.
· Complies with Home Health Compliance Plan, Infection Control and HIPPA policies
· Available to work after hours and weekends on an on-call basis, per policy to meet the client care requirements.
Working Environment:
Field visits in patient environments with occasional office duties as directed by supervisor. This is a responsible and relatively complex position requiring exercise of independent judgment, tact, technical knowledge, skill and initiative. Work is not directly supervised. Exposed to daily routine driving hazards and weather conditions. Position does expose incumbent to uncontrolled environments. Occupational exposures are inherent to this position during visits.
Continuing Education Requirements:
Agency personal are expected to participate in appropriate ongoing education and training as required by the Agency. CEU credits will be completed as required by State licensure.

Applies hand-washing principles during daily work; demonstrates understanding of isolation precautions; recognizes signs and symptoms of infection and complies with the employee health program; demonstrates understanding of the process for identifying and handling infectious waste; maintains personal hygiene; complies with OSHA standards in the work place; and demonstrates understanding of cross-contamination.
Information Management
Enters or records data timely and accurately; protects confidentiality of patient information; protects data against loss or destruction; reports suspected violation of security/confidentiality issues.
Position Qualifications:
· Licensed Registered Nurse in the State of Oklahoma
· Home Care/Case Management experience preferred
· Excellent communication and time-management skills with ability to work independently
· Knowledge of regulatory standards and compliance.
· Competency Requirements: proficient at venipuncture, CPR, infusion therapy services including intravenous medication administration, port access device care, basic rehabilitation therapy, and wound care to include negative pressure therapy
· Working knowledge of computer and software applications used in job function (Word, Excel, Outlook and EMR software)
· Current Automobile insurance coverage
· Workable knowledge of Medicare, CHAP accreditation criteria and standards
· Willing to be on call as assigned
