About the Role:

As a PRN Hospice Registered Nurse, your primary responsibility will be to provide compassionate and comprehensive care to terminally ill patients in their homes or hospice facilities. You will work closely with a multidisciplinary team to develop and implement individualized care plans, ensuring the physical, emotional, and spiritual well-being of patients and their families. Your role will involve assessing patients' conditions, managing symptoms, administering medications, and providing education and support to patients and their loved ones. Additionally, you will collaborate with physicians, social workers, and other healthcare professionals to coordinate and facilitate the delivery of high-quality end-of-life care.

Minimum Qualifications:

  • Valid Registered Nurse (RN) license in the state of Oregon
  • Minimum of 2 years of nursing experience, preferably in hospice or palliative care
  • Strong knowledge of pain and symptom management
  • Excellent communication and interpersonal skills
  • Ability to work independently and make sound clinical judgments

Preferred Qualifications:

  • Certification in Hospice and Palliative Care Nursing (CHPN)
  • Experience working with diverse populations and cultural sensitivity
  • Proficiency in electronic medical record (EMR) systems
  • Bilingual proficiency in English and Spanish
  • Experience in providing grief and bereavement support

Responsibilities:

  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Administer medications and treatments as prescribed, ensuring accurate documentation
  • Provide emotional support and counseling to patients and their families
  • Educate patients and families on disease management, symptom control, and end-of-life care options

Finally, your knowledge of pain and symptom management will ensure the comfort and well-being of patients under your care.
