The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE:

Provides expert, high-level knowledge of health care, technical skills, and analytical expertise in the care of patients undergoing transplants either BMT/CT or solid organ. Responsible for coordination of the evaluation and preparation of patients for transplantation, cellular therapies, and long-term management of patients post-treatment. Provides comprehensive coordination of patient care across multiple areas—the outpatient clinics, the inpatient nursing unit and the community—in order to provide continuity of patient care across settings. Completes a workflow that has a significant level of complexity due to both clinical and logistical issues. Applies appropriate interventions to individual patients and patient problems while performing daily tasks. Serves as a clinical resource for other team members, patients/family/caregivers, referring case managers, and community clinicians.

FT position available M-F, with some rotating overnight call taken from home managing the logistics of setting up kidney transplants.

This is an exciting opportunity to support kidney transplant recipients by providing high-quality care, education, and resources throughout their post-transplant journey. The coordinator plays a vital role in developing strong relationships with patients, their families, and outside providers, ensuring continuity of care and fostering trust. In this role, the RN will assess patient needs, triage concerns, provide clinical guidance over the phone, and coordinate with multidisciplinary team members as appropriate. Strong clinical assessment skills, attention to detail, and a passion for patient education are essential, as this position requires frequent patient communication and the ability to manage multiple priorities in a fast-paced environment. The coordinator will also participate in quality improvement initiatives, policy and procedure development, and committee activities while serving as a resource for both internal staff and external healthcare providers. Candidates must have excellent communication, organizational, and critical thinking skills, with prior transplant or complex care coordination experience preferred.
