Position Duties

  • Provides communication between all health care workers, residents, family, and the community both orally and in written format. 
  • Assesses the resident's condition on an ongoing basis utilizing the nursing process. Documents information gathered in accordance with the nursing process and facility policies and procedures. 
  • Evaluates resident's condition and identifies the need to notify the attending physician and family/legal party responsible of any change in the resident's condition. 
  • Administers medication and treatment per MD orders and provides direct resident care. 
  • Monitors use of special equipment, i.e oxygen, intravenous, etc.
  • Participates in the care planning process in coordinating nursing services with other discipline services. 
  • Directly responsible fore supervision of the daily work assignments, care plans, and performances of assigned activities of nursing staff, i.e LPNs, Attendants, Nursing Assistants, in accordance with the Nurse Practice Act. 
  • May prepare and implement employee performance evaluations for LPNs, Attendants, and Nursing Assistants. 
  • Understands and promotes the concept of comprehensive individualized nursing care for a diversified population. 
  • Consults with management regarding the discipline, promotion and discharge of assigned nursing staff. 
  • Initiates investigations of incident and accident reports of residents and employees, coordinating with Social Services.
  • Initiates and prepares updates to the resident plan of care. 
  • Assesses resident for any permanent change in condition that would result in an alteration of the care plan. Notifies Area Nurse Manager of such.
  • Initiates specialty assessments as needed, i.e safety, hydration, skin, pain etc.
  • Reviews and updates care plans for acute changes in condition, i.e UTI, URI, etc. in between quarterly reviews by RACs.
  • Evaluates Pharmacy indicators as appropriate for nursing judgement. 
  • Attends in-service education programs and provides on-unit orientation for nursing staff.
  • Provides facility updates to all nursing staff, i.e policy and procedure updates and changes. 
  • Attends in-services provided by facility in order to update their skills. The nurses also provide suggestions for topics they would like to have discussed at monthly meetings. 
  • May assume charge nurse duties to meet facility needs. 
  • Participates in the Quality Improvement Program. 
  • May perform other varied duties/responsibilities per facility needs. 

Minimum Qualifications

  • Graduate of an accredited/licensed school of nursing. 
  • Licensed to practice as a Registered Nurse in the state of Wisconsin as defined by S. 441.11 (1) Wisconsin Statutes. 
  • Previous work experience in geriatrics, medical, surgical, psychiatric, or with the developmentally disabled preferred. 
  • Ability to handle crisis intervention situations as directed by individualized care plan, facility protocol, resident rights, etc. 
  • Understands and effectively carries out oral and written instructions. 
  • Ability to plan and direct the work of others. 

Other Qualifications

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. 

This position is very active and requires standing, walking, bending, kneeling, stooping, crouching, crawling, and climbing all day. The employee must frequently lift or move objects and patients weighing over 50 pounds. 

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. 

Specific hearing abilities required by this job include, hearing conversation in noisy and quite environments, locating a sound source and differentiating among non-speech sounds, communicating through speech. 

Ability to adapt to changes in daily work hour and schedule. 

Initiate timely emergency resuscitation measures according to adult resuscitation and advanced life support protocols. 

Risk of blood borne pathogen exposure is considered high.
