What You’ll Do

  • Lead the MDS Process: Coordinate and oversee the Minimum Data Set (MDS) assessments for all residents, ensuring accuracy and timeliness.
  • Care Plan Development: Collaborate with the interdisciplinary team to create and maintain individualized care plans that meet resident needs and regulatory requirements.
  • Ensure Compliance: Maintain compliance with federal, state, and local regulations, ensuring documentation meets all CMS and DOH guidelines.
  • Resident & Family Communication: Serve as a resource for residents and families regarding care plans and assessment processes.
  • Data Integrity & Reporting: Monitor quality measures, review assessment data, and submit reports as required.
  • Team Collaboration: Educate and support nursing staff on accurate documentation and assessment protocols.
  • Quality Improvement: Participate in QAPI meetings and initiatives to continually improve resident outcomes and facility performance.

 

Qualifications

Required:

  • Current, valid RN license in Pennsylvania.
  • Minimum of two years’ clinical nursing experience in a skilled nursing facility or similar healthcare setting.
  • Working knowledge of the MDS process, RAI guidelines, and care planning in long-term care.
  • Strong organizational, documentation, and communication skills.

Preferred:

  • Experience as an RNAC or in a similar assessment coordination role.
