Job Summary:
The Senior Lead CNA - East Providence reports to the Senior Day Center Director in East Providence and is responsible for leading the day-to-day workflow and operations of Day Center staff. The role involves organizing and directing floor staff by coordinating CNA assignments, mentoring and training new staff, and ensuring timely communication of essential updates from Interdisciplinary Team (IDT) meetings to all relevant staff.
The Lead CNA – East Providence ensures the completion of CNA assignments, coordinates participant care with the health center, and works closely with the Day Center Nurse to address participant concerns and implement nursing recommendations, such as toileting schedules, hydration plans, or other care directives.
In the absence of the Senior Day Center Director, the Lead CNA – East Providence assumes the role of primary point of contact for any emergent operational issues. This includes ensuring that the day center runs smoothly and efficiently, maintaining continuity of care for participants, and addressing any staffing concerns or coverage gaps. The Lead CNA – East Providence will also manage the delegation of duties to ensure that both staff and participant needs are met during this period. Additionally, they may be responsible for providing guidance and support to the team, handling any immediate challenges, and ensuring the overall safety and well-being of the center's attendees.
Job Duties/Responsibilities:
- Ensures coordination of services in order to provide a calm, even flow of the PACE center by assigning staff to appropriate work assignments and rotating staff fairly.
- Creates work assignments for CNA staff and ensures their completion at the end of the shift. Reports discrepancies to the Senior Day Center Director.
- Immediately reports any high-risk issues to the Senior Day Center Director and addresses other concerns that require attention.
- Works to maintain an appropriate and positive atmosphere in the PACE day center, enforces the Declaration of Cooperation among staff and participants.
- Ensures the timely completion of vital signs, blood sugars, heights and weights as requested by clinical team and records appropriately in the EHR.
- Serves as a mentor and trains new CNA staff as well as provides on-going support to established staff.
- Assists in the interview process for new staff as appropriate and contributes to CNA performance evaluations yearly.
- Completes new staff competencies with the assistance of nursing, ensuring a thorough and timely orientation that complies with established guidelines. Reports any concerns or training needs to the supervisor.
- Serves as a bridge and resource between the Day Center staff and Life Enrichment team, coordinating efforts to ensure appropriate attendance and participation in events. Supports Life Enrichment activities as needed, encouraging socialization opportunities for clients and promoting engagement in these events.
- Works with the Day Center RN to identify the learning needs of the PACE center staff or in-service opportunities for the Organization to reduce risk or increase CNA competency and skills.
- Monitors food service for the appropriateness of client groupings during meals to ensure safety and socialization.
- Provides behavioral supervision and support for participants, monitors and responds to behavioral disturbances such as wandering, agitation, restlessness, exit seeking, etc. and enforces behavioral plans.
- Demonstrates strong leadership skills with the ability to mentor and support new staff.
- Maintains an organized approach to workflow and can adjust and adapt effectively to handling competing priorities.
- Ensures infectious disease control procedures are adhered to (such as sanitizing the day center) and that CNAs are wearing proper PPE at all times.
- Report any facility improvements or damaged items requiring replacement to the Senior If necessary, creates plans for staff to continue their duties while the item is being repaired.
- Provides assistance to management staff by providing support to staff, information regarding clients and day to day operational issues, participating in IDT meetings, reporting high risk issues immediately and maintaining a sense of order and flow in the PACE center.
- Completes the day center assessment tool for new and existing participants and presents findings in care planning meetings
- Maintains on-going and accurate participant lists for showers, toileting, diet, allergies and attendance according to participants care plan. Ensure accuracy of service summary in EMR
- Coordinates assistance with the Health Center for any medical or consult visits as requested. Provides staffing to health center when needed.
- Problem solves staffing issues between transportation, day center and health center and assigns staff to different departments as needed per skill level.
- Ensures CNA competency to work in the assigned area.
- Ensures the completion of the daily attendance sheet and assist in making daily calls when requested to ensure participants safety when call outs are received by reception. Ensure communication to the IDT and RN
- As needed provides in-home care including personal care and housekeeping services as assigned.
- Actively participates in team meetings to communicate relevant issues to the team and ensure all center staff are informed and up to date with current information from IDT meetings.
- Ensures staffing is maintained at appropriate levels for safety and secures and coordinates coverage as necessary.
- Works with operational managers of transport, food services and the pharmacy to assist with staffing coverage from Day Center staff pool as necessary and able.
- Ensures that documentation from team meetings is maintained and changes are recorded in individual progress notes and/or care plans as required.
- Serves as primary point of entry for ensuring participants are oriented to the program and have opportunities to express opinions about their care and services.
- Under the direction of the Senior Day Center Director communicates with participants, families, contracted providers, and the public regarding issues related to the routine operation of the Day Center.
- Attends team and Care Plan meetings. Reports necessary and relevant information back to Senior Day Center Director and floor staff as appropriate.
- Coordinates and oversees the production of staff, participant, visitor and student ID badges as well as insurance cards.
- Coordinates and leads daily meeting with CNAs to review day center operations and participant reports.
- Supports the Home Care Manager to conduct emergency home wellness checks as needed and as feasible.
- Accompanies participants to specialist visits as needed.
- Works rotating weekends and/or holidays as assigned.
- May be requested to work at other PACE-RI sites.
- Demonstrates an understanding of the importance of clear, thorough documentation as crucial to good participant care; completes and enters accurate, thorough participant documentation in the electronic health record; enters documentation for assessments, orders, and notes within required timeframes.
- Performs all other duties assigned by the Day Center Director.

Education Requirements:
High School Diploma or Equivalent, Required
Physical Requirements:
Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch and reach for extended periods of time.
Must be able to regularly lift objects up to 25 pounds and at times up to 35 pounds.
Must be able to assist a 200-250 lb. participant, using proper body mechanics.
Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants.
Must be able to navigate various departments of the organization's physical premises as well as related community care settings.
Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases.
Must be able to complete assignments in variable weather conditions and ever-changing and sometimes allergenic environments in participant homes, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies.
Experience:
3 years of CNA experience, Required
1 year of experience working with a frail or elderly population, Preferred
License & Certification Requirements:
Rhode Island Certified Nursing Assistant License (CNA)
Position Requirements:
CPR Certification
Driver's License & access to reliable transportation: community-based travel may be required.
