Definition

The Clinical Support Specialist is responsible for monitoring and coordinating patient visit schedules, ensuring the correct frequency of visits, and managing data entry into the electronic medical record (EMR) system. This role requires attention to detail, organizational skills, and effective communication with clinical leadership to ensure all visit requirements are met and documented accurately.

 

Qualifications

  • High school diploma or equivalent; Associate degree in healthcare administration or a related field preferred.
  • Previous experience in healthcare or clinical support, especially in scheduling and data entry.
  • Strong attention to detail and ability to maintain accurate patient records in an EMR system.
     Knowledge of clinical operations, visit frequency, and patient care requirements.
  • Proficiency with EMR systems and scheduling software.
  • Excellent organizational and communication skills, with the ability to work well with clinical leadership and staff.
  • Ability to identify and resolve discrepancies in visit data and schedules in a timely manner.

  

Key Result Areas

  • Visit Scheduling and Coordination - work with clinical leadership to schedule patient visits, ensuring the correct frequency of visits based on the patient’s plan of care.
  • Collaborate with clinical teams to adjust schedules as necessary, ensuring timely visits and resource allocation.
  • Coordinate with patients, caregivers, and clinical staff to confirm appointments and ensure clear communication regarding visit details.
  • Monitor and track visit schedules to ensure that no visits are missed or delayed.
  • Data Entry and EMR Management
  • Ensure that all patient visits, treatments, and assessments are accurately entered into the EMR system, following organizational guidelines for documentation.
  • Track and verify that the correct data (e.g., visit frequency, correct patient assessments) is recorded for each patient visit.
  • Perform routine audits of visit data to identify and correct any discrepancies in patient records.

 

  • Ensure that data entered into the EMR is complete, accurate, and up to date to support quality care and compliance.
  • Monitoring Visit Compliance: notify clinical leadership if there are discrepancies or concerns with visit schedules, documentation, or patient care requirements.  
  • Assist in ensuring that the correct treatment and visit frequency align with the patient’s care plan and organizational standards.
  • Collaboration and Support: Provide regular updates to clinical leadership on visit schedules, patient data entry, and any issues or delays in documentation.
  • Work closely with the clinical team to ensure all visits are scheduled and completed according to established protocols and patient care needs.
  • Support the clinical team by providing timely and accurate information on patient visit requirements and documentation needs.
  • This position may require weekend coverage and alternating schedules.

Position will be categorized as category III (involves no routine or potential exposure to blood, body fluids/tissues).

Additional Information

Incumbent may be required to perform emergency duty before, during and/or beyond normal work hours or days in the event of an emergency, crisis situation or disaster (man-made or natural) including evacuation sites. The person in the position will work in a smoke-free location and is expected to adhere to all smoking restrictions.
