Overview:What you can expect to do.
We are looking for a compassionate and organized School Case Manager to join our team. The School Case Manager will work closely with students, families, and school staff to provide support services for students facing academic, behavioral, or social challenges. The Case Manager will coordinate services and resources to help students achieve success and meet their educational and personal goals. This role requires collaboration, problem-solving, and advocacy to ensure that students receive the appropriate support and interventions.
Responsibilities:Student Support and Case Management:
- Work closely with students, parents, and school staff to assess individual needs and provide tailored support services.
- Develop individualized action plans to address academic, behavioral, social, or emotional challenges.
- Monitor and track student progress toward goals, making adjustments to support plans as needed.
- Coordinate services such as counseling, tutoring, and special education services for students with identified needs.
Collaboration and Communication:
- Serve as a liaison between students, families, school staff, and outside agencies to ensure effective communication and resource coordination.
- Lead or participate in meetings (e.g., IEP meetings, 504 plan meetings, parent conferences) to address student needs and progress.
- Work with teachers, administrators, and other staff members to implement strategies and interventions that support student success.
- Provide regular updates to families on student progress and resources available to them.
Referral and Resource Coordination:
- Identify students who may benefit from additional services, such as mental health support, behavioral interventions, or community resources.
- Refer students and families to appropriate outside agencies or community organizations for support (e.g., social services, mental health counseling).
- Assist families in navigating school systems and services to ensure students receive necessary accommodations or interventions.
Monitoring and Documentation:
- Maintain accurate, up-to-date records for all students receiving case management services, including progress reports, meeting notes, and action plans.
- Ensure that student information is kept confidential and in compliance with legal and ethical guidelines.
- Monitor the effectiveness of interventions and services, making necessary adjustments to ensure the best possible outcomes for students.
Advocacy:
- Advocate for students’ educational and personal needs within the school environment, ensuring they receive the appropriate support.
- Help students build self-advocacy skills by teaching them how to express their needs and access resources independently.
- Address barriers to learning and success, such as attendance, behavior, or family-related issues, by collaborating with relevant stakeholders.
Professional Development:
- Stay current on best practices, trends, and legal requirements in student support services and case management.
- Participate in ongoing professional development opportunities to enhance skills and knowledge.
- Work with school leadership to continuously improve the case management program and support services for students.
Other Duties as Assigned:
- Assist with school-wide initiatives related to student well-being, academic support, or crisis intervention.
- Participate in multidisciplinary teams and collaborate with other staff to address school-wide student needs.
Education:
- Bachelor’s degree in Social Work, Psychology, Counseling, Education, or a related field.
- Master’s degree in Social Work (MSW), Counseling, or a related field preferred.
Experience:
- At least 2-3 years of experience in case management, social work, counseling, or a similar role, preferably in a school or educational setting.
- Experience working with students from diverse backgrounds and those facing academic, behavioral, or emotional challenges.
Skills:
- Strong communication and interpersonal skills to build relationships with students, families, and staff.
- Excellent organizational and time-management abilities.
- Ability to collaborate effectively with multidisciplinary teams and outside agencies.
- Strong problem-solving and conflict resolution skills.
- Proficiency with case management software, educational platforms, and Microsoft Office Suite (Word, Excel, PowerPoint).
- Ability to run short distances and move swiftly in response to student needs, including bending, kneeling, and crouching.
- Ability to lift and assist students with mobility challenges, often requiring the handling of up to 50 pounds or more.
- Skill in managing fine motor tasks, such as helping students with writing, manipulating small objects, or using adaptive devices.
- Ability to operate a computer or tablet for up to 8 hours daily.
- Capacity to notice and respond to non-verbal cues from students
- Capacity to remain calm and composed during physically and emotional demanding situations, ensuring student safety and well-being
