HHA/PCA - Hultquist Place
Full-Time | 1st Shift (6:00 AM – 2:00 PM, Minimum 37.5 hours/week)
Schedule: Every other weekend and at least 1 day off during the week
Salary Range: $18.10 Minimum to $25.05 Maximum
Shift Differential: Lutheran employees scheduled to perform medication pass will receive an additional $1.50/hour.

What We’re Looking For:
High School Diploma or equivalent required
Current HHA or PCA certification from New York State required
Prior experience with medication pass is a plus -- this individual will be responsible for working on the medication cart
Basic computer skills and ability to use electronic medical record systems
Strong communication, observation, and time-management skills
Dependable, compassionate, and team-oriented attitude
What You’ll Do:
Provide personal care and support per individualized care plans
Administer medications (once trained and authorized)
Assist residents with hygiene, dressing, grooming, and ambulation
Monitor and report changes in condition to the RN Case Manager
Perform rounds, respond promptly to call lights, and ensure resident comfort
Complete electronic documentation accurately and timely
Collaborate with nurses, physicians, and other healthcare staff
Assist with meals, housekeeping, and other daily living tasks
Uphold resident rights and dignity at all times
Support residents through admissions, transfers, and discharges
Maintain a safe, clean, and welcoming environment
