The employee provides support to the home health aide team in the key areas under the direction of the VNHS Clinical Manager. This employee is a professional who has received certification training and is responsible for therapeutic care, personal care and incidental services under the plan of care established by the Clinical Manager. This employee provides patient-focused care utilizing the plan of care developed by the Clinical Manager to perform the duties and functions listed below. The employee will assist clinical manager with scheduling, client admission and client care.
Responsibilities:
- Perform safe and effective Client care procedures and techniques per care plan and scope of practice.
- Report unsafe conditions, changes in client’s physical or mental behavior or changes in the environment to manager.
- Perform personal care including bathing, hair care, oral care, skin care, nail care and other personal hygiene measures. This may include bowel care, digital stimulation, enemas and Ostomy care.
- Transfer clients, including those with mobility limitations, to and from bed or chair which may require the use of a Hoyer lift or transfer board.
- Assist with ambulation and range of motion exercises as directed by the RN Case Manager.
- Assist with medications that are ordinarily self-administered. Medication Aides may administer limited routes of medications per care plan.
- Perform incidental services for clients as specified on the Client care plan, including but not limited to, changing bed linens, mopping, laundry, light housekeeping.
- Receive, review and refer inquiries regarding the programs of the agency.
- Schedule appropriate personnel based on needs of clients.
- Update and maintain data within CHAMP software system.
- Report pertinent client and employee information as appropriate, involving management rapidly when there is a client complaint or other emergency.
- Respond promptly, professionally and courteously to all client and employee calls, using tact, diplomacy and professionalism to maintain the highest level of customer satisfaction.
- Serve as liaison between Home Health Aide and Clinical Manager.
Requirements:
- 2 years’ experience as a CNA or Home Health Aide preferred. High School Diploma required. Associate’s degree in business or related field preferred.
- Current CNA certification in the state or states where employee will see patients or satisfactory completion of a certified Home Health Aide course and 12 hours in-service training annually. Will complete and pass Medication Aide Training.
- Current CPR License Required.
- Office and customer service experience required, Senior Care experience preferred.
