To provide Basic Population Health Management services for members which includes engaging with members individually or as part of a group to support lifestyle modifications to manage chronic diseases, to identify care gaps and needed services, and to aid Partnership members in maintaining wellness. Healthy Living Coaches shall also serve as a resource about Partnership as well as Population Health programs and services.
Responsibilities:- Proactively engage and connect with Partnership members through multiple modalities that may include phone, web-based tools, group presentations, and/or at community events, per PHM leadership direction.
- Maintain records documenting outreach activities.
- Perform Basic Population Health Management activities for assigned member caseload to help members identify opportunities to improve and support healthy lifestyle change using motivational interviewing strategies.
- Refer members to appropriate screening tools, identify self-management tools, educational material, and special training needs. Monitor and track member use of referred resources and provide recommendations for resources needed. Document closed-loop referrals to community resources, providers, and other available services.
- Provide current information on Partnership’s benefits, services, updates, and wellness
programs, as appropriate. - Attend member-facing events to promote Partnership’s presence in the community while representing Partnership in a professional manner per Partnership policies. Gather member feedback, and identify other agencies that provide resources to the members we serve.
- Coordinate logistics of tabling assigned events including swag, incentives, member-facing materials, Partnership brand promotion (i.e. tablecloths, banners, etc.), and returning materials to Partnership inventory upon event completion.
- Gather information and provide feedback per departmental guidelines to leadership
regarding the barriers members face accessing wellness care. Provide recommendations on means to promote members knowledge and resources to health and wellness. - Work with Health Education to identify appropriate member educational materials to
share with community partners, providers, and members. - Other duties as assigned.

HIRING RANGE:
$72,364.92 - $90,456.15
