The Registered Nurse Assessment Coordinator assumes responsibility for the MDS, and how it relates to facility CMI, Quality Assurance and Interdisciplinary Care Plans. Coordinates Care Plan meetings and the communication with members of the team.

JOB DUTIES AND RESPONSIBILITIES:

  • Interviews new residents and gathers data for the MDS

  • Instructs other RN’s and LPN’s re: MDS and care plans

  • Completes computer data entry for MDS as required by state regulations

  • Maintains current knowledge of MDS and CMI by attending seminars

  • Assesses resident’s total care and coordinates care with the interdisciplinary team to improve CMI

  • Functions as a staff nurse on alternating weekends or PRN basis

  • Evaluates the quality and documentation on progress records and resident care plans and completes documentation as necessary

  • Assists in daily care of residents and feedings as needed.  Supervises licensed practical nurse and/or certified nurses aide

PHYSICAL AND SENSORY REQUIREMENTS:

Prolonged walking, standing, sitting, reaching and bending. Excellent speaking and hearing for effective communication. Near vision acuity for reading reports.

Ability to move throughout the facility with ease. Exposure to patient elements. Ability to withstand high stress situations and meet demanding deadlines.

Applicants will be evaluated through a pre-employment Physical Therapy Back School and Body Mechanics Screen test. 

EDUCATION:

Registered Nurse with a current Pennsylvania license required. CPR certified required.

TRAINING AND EXPERIENCE:

Previous experience in geriatrics working as an MDS coordinator with data entry knowledge preferred. QA experience and knowledge of CMI preferred.
