The Admissions Coordinator is responsible for managing referrals and facilitating admissions to Country Manor. This role requires respectful and professional communication with both internal and external referral sources, whether in person, by phone, or electronically. Key responsibilities include receiving and responding to inquiries from prospective residents, conducting pre-admission assessments at hospitals, and serving as a liaison between physicians, patients, referral sources, and Country Manor. The Admissions Coordinator will also oversee the verification of insurance policies for skilled nursing services, including checking eligibility, coverage, and deductible amounts. Ensuring that all insurance information is accurately documented and effectively communicated to the appropriate individuals is essential. The Admissions Coordinator must consistently reflect Country Manor’s Hometown Culture, values, and integrity in every interaction.

As an Admissions Coordinator at Country Manor, you'll:

  • Coordinate, respond to, and follow up on all inquiries about services at Country Manor.
  • Make Initial contact for admission and transfers to Country Manor.
  • Maintain current census of LTC facility by unit, room, and resident. Communicate open beds and types of beds available at Country Manor daily to referral sources.
  • Assist with collaboration with hospital social workers and discharge planners to facilitate and ensure seamless transfer to Country Manor.
  • Complete, record, and communicate results of pre-admissions surveys to assist Country Manor Staff in admission decisions.
  • Assist in promoting a smooth transition to Country Manor.
  • Assist with coordination with Country Manor nurses, social workers, and rehab staff to make admission decisions.
  • Assist/Facilitate admission process for Country Manor Health Care Center and Rapid Recovery & Aquatic Center. Make recommendations to streamline and improve processes, as needed, including use of forms, information, responsibilities, and procedures, etc.
  • Assist with interpretation and make available the required information and forms for each admission and for the responsible party.
  • Conduct or delegate tours and pre-admission meetings with prospective clients, their families, significant others, and/or referring parties on a timely basis.
  • Inform referral sources of additions, innovations, and program enhancements.
  • Manage your own time in order to accomplish assigned duties
  • Present professional image and dress consistent with facility dress code policy.
  • Assist in the promotion of programs and services including speaking engagements, trade shows, and other public relations activities. 
  • Participate in assigned meetings.
  • Comply with all administrative policies and procedures.
  • Coordinate time off with other entities.
  • Verify necessary pre-admission data i.e.: payment source, insurance authorizations, Medicare/MA#, client preferences, etc.
  • Obtain physician orders and determine necessary equipment needs and/or other requests.
  • Provide demographic information to supervisor
  • Complete other duties as assigned.

Required Qualifications:

  • Licensed Social Worker, Registered Nurse or Licensed Practical Nurse in the State of Minnesota
  • Must be skilled in oral and written communication
  • Ability to work effectively with other health care professionals.
  • General knowledge of Medicare, Managed Care, Medicaid and Commercial health plan reimbursement methodologies, and documentation requirements preferred.
  • Knowledge of costs associated with different levels of care.
  • Knowledge of and skill in using personal computers and related software for word processing, spreadsheets and clinical software applications.
  • Skill of developing and maintaining positive working relationships with clients and other employees; ability to respond with a sense of urgency to client’s needs.
