Salary Range: LPN - $31.00 - $36.25 Based on experience level

                          RN - $34.00 - $39.25 Based on experience level

Position Overview

As a Primary Care Nurse, you will work closely with providers, medical assistants, and multidisciplinary teams to deliver compassionate, coordinated care. This includes triaging acute illnesses, supporting patients during care transitions, managing medication refills, and providing education to help patients achieve their health goals. You will also be cross-trained in at least one of the following areas: Residency, Medication Assisted Recovery (MAR), Mobile Health Van, Prenatal, or Pediatrics.

🕑 Day shift only – no nights or weekends required.

Key Responsibilities

  • Collaborate with the primary care team to provide safe, high-quality nursing care.
  • Triage acute patient complaints, offer home care advice, and ensure timely care.
  • Support care coordination, quality improvement initiatives, and performance measures.
  • Provide patient and family education to promote self-management and advocacy.
  • Assist with health screenings, lab procedures, and patient intake/discharge.
  • Help identify and manage care for high-risk patients to improve population health outcomes.

Qualifications

  • Current New Hampshire nurse license (RN preferred, LPN accepted)
  • Experience in primary care or ambulatory health care setting preferred
  • Current BLS certification
  • Strong assessment, critical thinking, and organizational skills
  • Ability to work collaboratively and adapt in a fast-paced, diverse setting
  • Comfort with electronic health records (Athena or similar) and telehealth platforms
