The Senior Registered Nurse – Mobile Outreach Coordinator’s main responsibility is performing nursing care and services at clinics, home visits, and community outreach events with a focus on promoting health and disease prevention. This position is also responsible for managing all off-site and mobile immunization clinics, coordinating all aspects of the clinic, and working closely with the host site to ensure that all offsite clinic locations have sufficient clinic items to operate smoothly and efficiently.

Position Duties

  • Cultivates and maintains positive relationships with external organizations, partners, and community members serving as a liaison between the program and external entities.
  • Plans, organizes, and implements outreach events, programs, or educational initiatives.
  • Plans, organizes, and manages off-site clinics with schools, detention facilities, and other community partners.
  • Analyzes data to measure effectiveness of outreach and uses data to adjust strategies for improved outcomes.
  • Tracks data for special clinics - number of requests, fulfilled or not, number of staff hours for each clinic, number of patients, number of vaccines, additional data for SF&T.
  • Updates off-site procedure documents and communicates changes with staff.
  • Manages publicly available email address to answer client and partner questions.
  • Works directly with schools and childcare facilities to ensure compliance with State reporting requirements as well as provides education and consultation on immunization related inquiries.
  • Provides support and training for county health care providers with Vaccines for Children (VFC),
  • Colorado Immunization Information System (CIIS) utilization, understanding, and ability to train relevant partners in use.
  • Provides direct care services (comprehensive knowledge of vaccine schedules and recommendations)
    • As stipulated by grant deliverables, actively participates, and provides patient care and consultation via community outreach at indoor and outdoor clinics, events, and fairs.
    • Lead for off-site events.
    • Shots for Tots and Teens monthly Saturday clinic Lead.
  • Provides outbreak emergency response.
    • Activated in the event of a public health emergency (typically to lead mobile operations for any vaccine response)
    • Completes sample collection (blood draws and/or skin, nose, and throat swabs) as needed for communicable/infectious diseases.
    • Completes home, hospital, off-site visits for urgent vaccine and IG administration as needed.
  • Works on special projects as assigned.

Emergency Preparedness and Response
  • Responds, as required, to support public health emergencies, incidents, and events. Employee participates in all exercises and drills on emergency preparedness and response, as required.
  • Completes trainings identified as appropriate for this level employee and initiates corrective actions and responses pursuant to Federal, State, and Local laws, statutes, and regulations.

Skills, Abilities and Competencies:
  • Inter-related professional services and resources in the Public Health environment and community for the implementation of programs.
  • Apply consistently and appropriately the principles, techniques, and practices as they relate to employee health, including the ability to read and analyze medical records as they pertain to immunizations and health risks
  • Relates well to all kinds of people, builds rapport, and establishes trust and confidence
  • Strong critical thinking and proficiency in the principles, techniques, practices, and protocols related to public health
  • Excellent verbal and written communication skills in working with healthcare professionals, administrators, employees, and environmental health and safety staff
  • Advanced skills in Microsoft Office, including the ability to write policies and procedures in Word, create and maintain spreadsheets with formulas in Excel, create presentations in PowerPoint, and use Outlook daily for email and time management. Proficient in electronic medical record documentation and use.
  • Expert knowledge of operational characteristics, services and activities of a comprehensive occupational health program in a healthcare setting and/or local public health department setting
  • Ability to determine when an MD or clinical supervisor needs to be alerted to situations, and which circumstances need medical advice before proceeding with action. 
  • Ability to use critical thinking skills and nursing expertise to effectively assess employees and care for their health to mitigate county risk.
  • Ability to create training relevant and pertinent to a wide variety of public health employees with varying levels of prior work experience.
  • Ability to uphold a strict code of ethics and ensure adherence to patient confidentiality standards and employment laws.

Minimum Qualifications

Education and Experience:

  • Bachelor’s degree in Nursing (BSN) from a school accredited by the National League for Nursing Accrediting Commission or the American Association of Colleges of Nursing.
  • Professional Qualifications: Licensed to practice as a registered nurse (RN) in the State of Colorado or a
  • Compact State. BLS and CPR certification.
  • 6+ years of nursing, public health nursing or related community or clinical work
  • Any equivalent combination of education and work experience that satisfy the requirements of the job will be considered

Other Qualifications

Pre-Employment Additional Requirements:

  • Must successfully pass pre-employment testing which includes an acceptable motor vehicle record (MVR) and background check.

Post-Employment Additional Requirements:
  • Must stay up to date on any required training, certifications, and licensures.

WORK ENVIRONMENT:
  • Work is primarily conducted indoors with occasional travel to other TCHD locations or outside agencies to provide program services or attend meetings. Some program positions require frequent travel to client residences. In the course of traveling to and working in other locations may experience occasional exposure to noise, dust, solvents, fumes, smoke or gases, slippery/uneven walking surfaces, and sun. Continuously works closely with co-workers and clients, and occasionally works alone. Frequently exposed to air/blood borne pathogens as a result of continuous contact with the public and children.
PHYSICAL DEMANDS:
The following are some of the physical demands commonly associated with this position.
  • Manual dexterity for performing exams and completing charts.
  • Frequent and continual driving, walking, sitting, standing bending/stooping, twisting, kneeling, and squatting.
  • Frequent light lifting/carrying of equipment/supplies and infants (under 15 lbs.), and occasional to frequent moderate lifting/carrying (15 – 49 lbs.) infants and small children.
  • Requires sufficient hearing and speech ability to communicate in person and via phone to respond to inquiries, and present information to individuals and groups as well as communicating important topics through language translator services. 
  • Reading comprehension is necessary to understand client records and medical documents.
  • Position requires continual and extensive driving to client residences and other community-based settings, including high risk (increased crime) neighborhoods. In the course of traveling to and working in other locations, may experience exposure to noise, dust, solvents, fumes, smoke or gases, slippery/uneven walking surfaces, sun, road hazards, traffic considerations, motor vehicle accidents, and exposure to inclement weather such as driving in snow, ice, etc. Potential for exposure to community related crimes while on duty.
  • Work conducted primarily indoors, such as client home environments which can be unpredictable, with potential exposure to environmental allergens, smoke, pets, vermin, bed bugs, cockroaches, potentially hostile clients or family members. Occasional client contact will occur outdoors.
  • High possibility of exposure to air/blood borne pathogens, insects, vermin, and other household and nature animals, as a result of continuous contact with the public, in community-based environments.
  • Potential exposure to high risk and emergent situations i.e. Client with suicidal ideation, child abuse or domestic violence situations, aggressive clients/family members/pets.
  • At high risk for compassion fatigue secondary to provision of care for high risk, complex clients and family situations. Frequent exposure to highly intense emotional situations such as client disclosures of current or past intimate partner violence, child abuse, or other traumas, potential exposure to situations of infant mortality/infant or fetal loss, etc.
  • Must use PPEs
