Description
- Engages clients and establishes trusting, collaborative relationships. Provides individualized supports to assist clients in obtaining their goals.
- Provides outreach services as necessary to clients when they appear to disengage from the service. Uses a variety of methods to provide outreach.
- Attends and participates in all required staff meetings and communicates individually with team members between meetings in order to coordinate needed services.
- Create timely, accurate and detailed notes to effectively document services delivered and to support billing activity and integrated case management.
- Works effectively and non-defensively with a variety of personalities, is aware of and sensitive to the cultural diversity of clients, co-workers, and other persons in the performance of job duties.
- Completes community job development tailored to client preferences
Qualifications
Education, Training & Work Experience: Experience/knowledge working with adults and children with mental health and behavioral struggles preferred. Maintain annual training hours required by FCC.
Specialized Knowledge & Skills: Trained in trauma informed care principles, Non-violent Crisis Intervention, skills-based interventions.
Equipment & Applications: Capable with Microsoft Office and Gmail/Calendar. Valid driver's license.
Work Environment & Physical Demands: General office setting conditions, to include work in client homes. Some local travel by car is required. Ability to maintain CPI principles if needed in a crisis situation.
