About the Department

This is a professional position involving responsibility for evaluating, planning and providing physical therapy treatment to patients in accordance with written prescription or referral from a physician who provides medical direction.  The Physical Therapist is responsible to a Senior Physical Therapist or, if there is no senior position, to a designated administrator/director for the operation of the department.  Supervision may be exercised over the work of Physical Therapy Assistants, Physical Therapy Aides and clerical employees assigned to the service.  A Physical Therapist does related work as required.

Position Duties

  • Evaluates the degree of function and disability of residents unless contra-indicated by professional judgment or a doctor's admission notes;
  • Develops a treatment plan based upon his/her findings, short and long term rehabilitation goals and in accordance with the physician's written order or referral;
  • Performs physical therapy procedures upon the written order or referral of a licensed physician;
  • Records on residents' charts all physical therapy evaluations, reevaluations, progress notes, consultation and discharge summaries;
  • Participates with other personnel in the formulation of residents' total care plans;
  • Maintains program records including but not limited to the service budget and statistical reports of the frequency and types of treatment and number of residents treated;
  • Confers periodically with a supervising Physical Therapist and physicians regarding all facets of resident care and related activities;
  • Develops or assists in the development of physical therapy policies and procedures;
  • Participates in the selection of personnel in the physical therapy services, assigns their duties and provides supervision;
  • Assists in the development of, instructs and participates in the facility in service education program; and
  • Performs other related duties as required.
FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS:  Good knowledge of the principles, techniques and practices of physical therapy; good knowledge of the operation and maintenance of apparatus and equipment used in physical therapy; good knowledge of pathological conditions and socio-economic factors affecting a resident's disabilities; ability to evaluate, plan and execute treatments for patients; ability to plan and supervise the work of others; ability to get along well with others; physical condition commensurate with the demands of the position.

Applicants must meet all requirements for one of the following qualification options in order to be approved for examination and/or appointment:

  • Master’s degree or higher in physical therapy[1].
  • A New York State Education Department permit to practice as an Physical Therapist.

License/ Certification 
  • Licensed and currently registered by the New York State Education Department as a Physical Therapist.
  • (1)  If the appointing authority is a:  (a) public,  incorporated or licensed proprietary hospital, a licensed nursing home, a public health agency, a recognized public school setting, or in the civil service of the state; AND 
  • (2) The appointing authority has a licensed physical therapist who is available and willing to provide on-site supervision as described in Education Law section 6735

[1] A Bachelor’s degree is acceptable, if license was obtained prior to the current mandates.

Other Qualifications

  1. Appropriate licensure must be maintained throughout employment.
  2. Limited Permitee’s appointed under Option 2 will be required to obtain their license in accordance with NYS Education Law Article 136 §6735.
PROVISIONAL APPOINTMENT NOTICE:
An individual who receives a provisional appointment must take the examination for the position title on the next occasion it is offered. In order to be eligible for a permanent appointment to the position, the provisional appointee must pass the exam and be ranked 1, 2, 3, or be tied in score with the individual ranked number 3 of those who are willing to accept appointment to the position. It should be understood that even if the provisional appointee is eligible for appointment, the employer may choose another candidate to fill the position on a permanent basis. Questions regarding provisional appointments should be directed to the Livingston County Human Resources Department.
