Summary:
The Regional Social Services Director provides leadership and support to social services departments across multiple centers.
Qualifications:
Education:
BSSW, MSSW preferred.
Licenses/Certification:
Licensing as required by the state of employment.
Experience:
at least Five year of experience in a long-term care environment, or equivalent.
- 3 year experience in a regional social service postition prefeered
Job Functions – Regional Social Services Director
Provides leadership, guidance, and oversight to facility Social Services Directors and Social Services Assistants across assigned centers.
Ensures psychosocial interventions and assessments are completed timely and in compliance with federal, state, and company standards.
Reviews care plans and progress notes to ensure quality, accuracy, and consistency across facilities.
Coordinates and supports facility teams in connecting residents with outside services, including dental, optical, and specialty providers.
Assists facility leaders in supporting residents’ families with transitions to skilled nursing, physical illness and disabilities, and the grieving process.
Partners with interdisciplinary teams to establish effective discharge planning processes region-wide.
Develops and delivers in-service programs to educate facility staff on psychosocial issues, patient rights, and best practices.
Coaches, mentors, and develops social services staff to promote professional growth and high-quality resident care.
Conducts audits, provides feedback, and implements action plans to improve overall social services performance.
Performs other regional leadership tasks as assigned.
Knowledge/Skills/Abilities:
- Ability to work cooperatively as a member of a team.
- Ability to communicate effectively with residents and their family members, and at all levels of the organization.
- Ability to maintain confidentiality.
- Knowledge of psychosocial practices applicable to a long-term care environment.
- Skilled in directing and motivating the workforce.
