Service: Direct Care Coordinator
Minimum Qualifications:
        Education:   Bachelor’s degree or High School Diploma/GED

Experience:  Four years of experience in related field or education or a combination of education and experience.

Other:  At least 18 years of age, dependable transportations, valid Florida driver’s license and proof of insurance. 

Purpose:
Direct Care Coordinators will provide training and assistance to all staff members, to include all direct care staff. The objective of the DCC is to promote, enforce, and monitor quality services rendered by all supports employed by Miracles In Motions. In addition, the Direct Care Coordinator could carry a case load of at least 3. 

Required Hours: Hours cannot be determined as they will change according to customers’ needs.  However, typically DCC hours of operation are Monday through Friday between the hours of 8:30am and 5:00pm. DCC’s are also expected to maintain a weekly rotating schedule with the team of coaches and management for the 24-hour on call services. In addition, DCC’s are expected to be on call at all times, to the agency emergency phone for potential questions or assistance needed. 

Duties and Responsibilities: Will include but not be limited to:
  • Random visits with the customer, while the employee is working to ensure that schedules are being followed and that quality services is being rendered.
    • These visits should be unannounced to the employee.
    • Visits are also to be done at a minimum of two times a month. However, there may be an increase of visits depending on issues that may arise related to the customer or client.
  • Managing and maintaining all staffing schedules.
    • As well as updating the Management Team of these changes
  • Inspect customers home to ensure all safety needs are met and maintained, including staff living area. (Detector, carpets, utilities, walls, odors, entrance way, etc.)
  • Ensure that all clients are being treated with respect and dignity by all staff personal.
  • Ensure that all staff personal is following doctor’s orders (diet, exercise, medication, etc.)
  • Create all necessary tools to ensure successful maintenance of all job requirements.
  • Conduct and maintain responsible, professional communication with all members of the community, family members, support coordinators and all others involved in the customer’s daily life.
  • Attend all required training (approximately 12 hrs. per year), staff meetings, and support plan meetings upon request.
  • Provide feedback and consistent communication with the Management Team.
  • Conduct all responsibilities in accordance with Miracles In Motion, Inc policies and procedures as they are written.
  • Any other duties related to the safety of the customer while they are involved as a companion, and any other duties assigned by the Executive Director.
